The undersigned is the parent of legal guardian of the Minor's name listed above (hereinafter referred to as "Minor"). The undersigned desire for said minor to attend or participate in ministries, events, programs, function and activities (hereinafter referred to as "Activity"), sponsored by, connected with, or related to Second Baptist Church Kalamazoo (hereinafter referred to as "Church").
Disclaimer: I understand and acknowledge that Second Baptist Church Kalamazoo will not allow the minor to participate in any Church activity without written consent and will not hold the church liable for any harm arising out of the minor's attendance and/or participation in any activity, program, and transportation sponsored by the church.
